Advance Care Planning in children’s palliative care. Can you help?

Karen Carr, a palliative care children’s nurse specialist from the University of Ulster, is researching health care professionals initiation of children's Advance Care Planning. 

She is trying to identify any grey evidence (papers, guidance, toolkits etc. produced by organisations outside of the traditional commercial or academic publishing and distribution channels), relating to the initiation of Advance Care Planning in children’s palliative care.
