Extract from the Foreword: 'While there have, for example, been marked improvements in open visiting and overnight accommodation for parents and carers, certain ward procedures and facilities for children remain suboptimal.
'Where the apparent failure to meet the standards of the relevant EACH Charter article is attributable to physical constraints, such as lack of space, we have flagged this up. However, many of the failures appear to be due to a failure to implement Scottish Government policy, such as having a dedicated named nurse or giving young people a say in the type of ward to which they are admitted.
'The problem is compounded by an apparent failure to monitor implementation and a lack of accountability.'
