WCDS TAC provides an individual and evolving collaborative team of the few key practitioners who see the child and family on a regular basis to provide practical support.

Providing a service for babies and young children who have a disability, or potentially disabling condition, or significant developmental delay, requiring 3 or more services to be involved and aged from birth to 5 years
